End Of Life Care

End of life care is support for people who are in the last months or years of their life. The care team have all been successfully trained under the 6 steps to success end of life programme.

End of life care should help you to live as well as possible until you die and to die with dignity. Allandale is committed to ensuring this is a peaceful journey whereby your wishes and preferences are taken these into account as we work with you to plan your care. Allandale is further committed to support your family, or other people who are important to you.

People who are approaching the end of their life are entitled to high-quality care, wherever they’re being cared for.

Good end of life care is tailored to the person who needs it. You and the people close to you should be at the centre of decisions about your care.

It helps if your wishes are written down as a personalised care plan and, if you agree, that this can be shared with the people involved in your care now and in the future. It’s important your care plan is reviewed regularly so that it stays up to date as your situation and wishes change.

The staff caring for you will show respect for you and behave kindly. Your comfort and dignity are important. That means, for example, that in the last days of life you can decide whether you feel like eating and drinking even if your need for food and fluid is already being met another way, such as through a feeding tube or a drip.

The Six steps that Allandale Care Group follow are.

  • Step 1  Discussions as end of life approaches
  • Step 2  Assessment, care planning and review
  • Step 3  Coordination of care
  • Step 4  Delivery of high quality care
  • Step 5  Care in the last days of life
  • Step 6  Care after death
